Personal Support Systems

Information is available for the asking. We urge you and your family to plan ahead for medical emergencies, establish a living will and designate a health care proxy for each family member.    Forms for Health Care Proxy designation can be printed off our web site (see Forms), or picked up at the front office. Organ donation, Living Will, and Out of Hospital Do Not Resusitate forms are also available from our office staff.

As specialists in family medicine, we are qualified to provide care for many of the medical problems you or your family will encounter. In those instances when consultation with another specialist is indicated, we will help you make those arrangements. This ensures that the consulting physician will contact your primary doctor with his or her findings.
